Contact urticaria due to chestnut(Castanea crenata)


Abstract
Contact urticaira due to vegetables and fruits is not uncommon, especially in atopic indivi¡þduals. Many patients usually do not consult to a doctor because most common symptoms are itching in the pharynx and transient mild angioedma of the lips. However, sometimes it produces respiratory symptoms, rhinitis and even anaphylaxis. Chestnut (C. creanata) is eaten among Koreans as raw state as well as boiled and baked. Koreans eat raw chestnut during drinking and at the time of ancestral sacrifice under Confucional custom.
A case of contact urticaria patient was observed who had an asthma history when he was a child. It was proved by scratch chamber test with-raw chestnut and Finn chamber on Scanpor tape. He was negative to all other extractants with water, alcohol, acetone. Boild or baked chestnut did not bother him.
The serial mean diameters of wheal between 1 : 100 histamine and raw chestnut w is a little different in that the wheal of raw chestnut appered a little slowly and persisted more long time than raw chestnut. The passive transter test with the patient¢¥s serum to normal person was negative.
